ACCIDENT/INCIDENT REPORTING
When an accident results in serious injury or death, or the vessel
has been damaged enough to make it unseaworthy or unsafe, the owner or
skipper must report full particulars of the accident or fire to the
Department within seven days. You can pick up a special form from the
Department for Planning and Infrastructure offices, or phone (08) 9216
8999 for one to be sent to you or

Marine safety complaints (speeding, skiing etc.)
Marine safety complaints (speeding, skiing etc) may be made to the State
Marine Operations Centre on telephone 9431 1000. The Centre is available
between 8.30am and 4.00 pm Monday to Sunday. Where required, a written
complaint may be forwarded to the Marine Safety Investigations Unit by
downloading the Marine Safety Complaint Report.
